Exempt Retirees Vision Program

What type of vision coverage is available to exempt retirees?

BNSF offers two levels of Vision coverage through EyeMed. Both coverages have the same basic features. The Premium plan allows you to get new frames and contact lenses every 12 months while the Basic Plan allows these expenses every 24 months. The plan provides for a copayment for examinations, lenses, frames, and contact lenses when you use an in-network provider. You can use other providers if you wish. When you do not use a network provider, there is a dollar limit to the coverage for each item. Consult the Summary Plan Description for the amount of copayment or Out-Of-Network dollar limits.

How do I get an ID card?

ID cards are mailed to participants in the program at initial enrollment. If you need a new ID card, call EyeMed at 1-800-521-3605.

Do I have to use a network provider in the vision plan?

No, you do not have to use a network provider. If you use a network provider, you will not have to file a claim form and will only pay a copayment for services (certain in network services do have restrictions). If you use an Out-Of-Network provider, there is a dollar limit for the service and you must pay for the services and submit a claim form for reimbursement.

How do I find a list of providers?

You can access a list of EyeMed providers through their Web site at www.eyemedvisioncare.com or by calling the customer service number at 1-800-521-3605.

How do I submit a claim?

If you use an Out-Of-Network provider, you must pay for the services first then contact the customer service number (1-800-521-3605) to determine where to send copies of your receipts for reimbursement or use the Claim Form below.
