Didn't Receive Your 2008 Safety Plate?

Safety plates were mailed to employees' homes starting in August and should now be in homes.

If your plate hasn't arrived, please fill out this form and supply all information requested. The form can also be used to request a replacement if your plate was broken.

To be eligible for a 2008 safety plate, employees must meet the following criteria.

• Employee must have worked at least three consecutive full months of the year of the safety plate award year.
• Employee must not have a reportable injury in the year of the safety plate award.
